The copy in hand also has a preliminary leaf with a note dated 22 June 1642, licensing Lawrence Blaiklock "to make two impression of the Booke of Rates", signed "Hen. Elsynge Cler. Dom Com.", and stating "This Booke of Rates according to this order is revised and examined by Robert Pye. Iohn Harison. Tho. Hoyle. Giles Greenel Sam. Vassall. Iohn Rolles."
