Note
Griffiths, in The bibliography of the Book of common prayer (1723/5), notes that gathering Fff (i.e. the first gathering signed Fff, following gathering Eee) has two leaves, Fff1-2. In this copy, there is no leaf Fff2, and the text is continuous between Fff1 and Kkk1.
Note
Unsigned inserted leaf after C4 (perhaps from Fff); recto = 'A prayer for the chief governor or governors of Ireland'; verso = 'Prayers for the use of Trinity-College, near Dublin.' and catchword = 'A form'. From the list of contents on leaf Pi2v, it would seem that this is part of gathering Fff.
Note
Contents list on leaf Pi2 verso has 'Irish' wording for items 33 ('Chief governor') and 36 ('Trinity College'), and pasted-in slip at foot of page with additional items 37-41 ('Act for the better observation of the Lord's Day', 'The thirty nine articles', 'Constitutions and canons ecclesiastical', 'The new canons' and 'Form of prayer for the visitation of prisoners').
Note
ESTC and Griffiths note the existence of a frontispiece; according to Griffiths it is by J. Gwin after Loggan.
