'These following returns were drawn up by Mr Alford Chapter Clerk - his clerk Mr Smith - and by myself in the year 1849 (the year of the visitation of the Cholera) and were copied [?] into this book September 1850 in the Muniment Room W K Hamilton (Precentor of Sarum and Can[on] Res [identiary]) Oct 1 1859.'
'I have given this book to the Dean of Salisbury this day 8 Sept 1850 with a request that it may be eventually be placed in the Muniment Room of the Dean & Chapter WK Sarum.'
